The clinic at 36 Toronto St. Suite 700A is a 4 minute walk south of St. Michael's Hospital at the corner of Victoria and Adelaide. It is about 100 m. from the subway station.
Duration
Initial visits are approximately 1.5 - 2 hours.
Please bring with you
- your valid health card
- a list of your medications, naturopathic and homeopathic supplements
- copies of any relevant test results
- the names and mailing addresses of physicians you wish reports sent to
Cancellations/Rescheduling
If you have to reschedule your appointment, we would appreciate as much advance notice as possible, to allow us to offer your time slot to another patient. However, we require a minimum of 24 hours advance notice for cancelled or rescheduled appointments. Patients who do not provide advance notice of cancellations will not be rescheduled.
Cancellation List
Patients must be available on short notice (3 - 4 hours) in order to be considered for a cancellation time slot. If you wish to be placed on the cancellation list, please inform the office staff and leave a contact cell phone number. If a time slot becomes available at short notice, a text message will be sent to all individuals whose names appear on the cancellation list. The time slot will be given to the first available respondent.
Test Results
Our office experiences a very high call volume. As a result, Dr. Vadas does not provide test results, medical advice or prescription refills by phone, fax or email. Patients may schedule an appointment with our office for these services or contact their family doctor.
Email Correspondence
Email correspondence is not secure and cannot guarantee confidentiality of health information. For this reason, Dr. Vadas will not provide test results, medical advice or discuss medical conditions by email.
Reminders
Appointment reminders may be sent be voice, email or text message. At the time of your initial booking, you will be asked for your email address or cell phone number for the purpose of a reminder call or contact. You will note two features on the email reminder: an option to add the appointment to your calendar program and a link to Google maps for our office location.
